Grade Change Form (After completing the form, click the printer icon at the top right of the screen. Extend the list to until you find an option that says "Print to PDF” or "Microsoft Print to PDF" from the Destination dropdown box and then click on Save. You must click on a "Print to PDF" option and not "Save as PDF." Email the saved PDF to judean.howerton@okstate.edu (ignore the instructions at the top of the form).
